Terms & Conditions: Fellowship in Reproductive Medicine (FRM)
Welcome to the Fellowship in Reproductive Medicine (FRM) program offered by Origins Hospital.
By applying for, registering, or enrolling in the FRM program, you (the "Applicant" or "Fellow")
agree to be bound by the following Terms and Conditions. Please read them carefully.
1. Eligibility and Admissions

1.1 Qualifications: The FRM is a specialized medical training program.
Applicants must hold a recognized medical degree, specifically MS (OBG), MD (OBG),
DNB (OBG), or DGO.

1.2 Admission Rights: Admission into the program is strictly merit-based
and at the sole discretion of the Origins Hospital selection committee. The Institute
reserves the right to reject any application without providing detailed justifications.

1.3 Documentation: All submitted credentials and certificates must be authentic.
Any discovery of forged or misrepresented documents will result in immediate termination
from the program and forfeiture of all fees paid.
2. Fee Structure and Conditional Scholarships

2.1 Base Course Fee: The standard program fee for the 12-month FRM
is INR 6,00,000 (Six Lakhs).

2.2 Inaugural Welcome Offer (Conditional Waiver): A 50% fee waiver is
available to the first 10 to 15 registered candidates on a first-come, first-serve basis,
bringing the effective payable fee to INR 3,00,000 (Three Lakhs).

2.3 Condition of Scholarship: This fee waiver is strictly a performance
and completion-based scholarship. It is entirely contingent upon the Fellow completing
the full 12-month tenure of the program and fulfilling all academic requirements.

2.4 Seat Booking: An upfront, non-refundable payment is required
to lock a seat in the program.
3. Ironclad Withdrawal & Discontinuation Penalty

3.1 Commitment: By accepting admission, the Fellow commits to the full
12-month duration of the program. Because seats are highly limited, blocking a seat and
subsequently withdrawing causes significant administrative and academic disruption.

3.2 Revocation of Waiver: If a Fellow decides to quit, withdraw, or discontinue
the course at any point before the completion of the 12-month period for any reason,
the 50% Fee Waiver is immediately and permanently revoked.

3.3 Financial Liability: Upon early discontinuation, the Fellow becomes
instantly liable for the Full Base Course Fee of INR 6,00,000.

3.4 Settlement of Dues: The Institute will calculate the amount already
paid by the Fellow and deduct it from the INR 6,00,000 base fee. The Fellow is legally
obligated to clear the remaining balance prior to being relieved from the Institute.
No experience letters, relieving orders, or logbook sign-offs will be provided until
all financial dues are settled in full.
4. Academic and Clinical Compliance

4.1 Rotations: Fellows must complete rotations as assigned across
various Institute branches (including but not limited to Hanumakonda, Khammam,
Sathupally, and Hyderabad).

4.2 Log Book Maintenance: The Fellow is mandatorily required to maintain
a daily Log Book documenting all cases observed, procedures assisted, and procedures
performed. This must be signed by the assigned mentor on a weekly basis. Failure to
maintain this logbook may delay or invalidate the completion of the fellowship.

4.3 Professional Conduct: Fellows must adhere to the highest standards
of medical ethics and follow all internal policies of Origins Hospital.
5. Intellectual Property and Confidentiality

5.1 Proprietary Information: All training materials, clinical protocols,
patient data, software access, and training on the proprietary "Origins Methodology"
provided during the fellowship are the exclusive intellectual property of Origins Hospital.

5.2 Non-Disclosure: The Fellow agrees to maintain strict confidentiality
and shall not reproduce, distribute, or share any of the Institute's proprietary materials,
success rate data, or operational software outside the organization, either during or after
the completion of the program.
6. Modifications and Jurisdiction

6.1 Amendments: The Institute reserves the right to amend these Terms &
Conditions, curriculum structure, or rotation schedules as deemed academically or
administratively necessary.

6.2 Governing Law: These terms are governed by the laws of India.
Any disputes arising out of or in connection with the FRM program shall be subject
to the exclusive jurisdiction of the courts located in Telangana, India.
